What to expect.

 

Your first visit

Our initial visit will be about getting to know you. We will talk about your medical history and current issues you would like to address. This first visit is more evaluative, though we will start acupuncture and get you comfortable with the process and how it feels.

When scheduling online, even for new patient visits, please book 4-5 return sessions out from your initial or return visit. You may book once every week or every two weeks if you are new and if we have not yet established a timeline schedule. This way you have sessions on the books, and we then won’t get behind the schedule. We can easily cancel any sessions we don’t need. You can check both Fremont and Green lake locations by clicking the location tab when scheduling. Thank you and I look forward to our session!

Intake Forms

Before your treatment can begin, it is important that the following forms are filled out before you arrive.

Please download and complete the Consent Form, Financial Agreement, HIPPA Release and Patient Insurance forms.

Making payments

Invoices will be emailed to you once your insurance has processed your claim. Payments can be made online by using Paypal. If using out-of-network insurance plans, you may pay via cash, check or credit card at the time of service, and I will provide a super bill/receipt for reimbursement.

CASH RATES:
Initial session: $155
Return sessions: $125

 

Insurance

I accept Regence, United, Aetna, Kaiser Permanente PPO and Personal Injury Protection/PIP cases (car accidents).

I welcome all other insurance companies as out-of-network and will send you a superbill for you to submit for reimbursement.

I recommend calling your insurance company to verify your acupuncture benefits and to ensure that I am in-network.

Cancellations

You may cancel 48 hours prior to your scheduled session. A $75 fee will be charged for late cancellations or no-shows.

Your Safety

In accordance with the Washington State guidelines, masks will be optional beginning April 3, 2023. You may request I wear a mask if you feel more comfortable and I will be happy to accommodate you. If you are sick, please notify me as soon as you are able so we may prevent the spread of illnesses. Thank you so much.
