Covid-19 Guidelines

Please take the time to carefully read the guidelines below:

Are you showing symptoms?
Please reschedule your appointment if any of the below applies to you.

• You have a fever, cough, muscle aches, sore throat

• You have been traveling in the last 7 days and are showing new symptoms

• You have been exposed to others who are sick or tested positive for a spreadable illness

When you arrive:

Fremont: You may wait on the black couch just past the drinking fountain on the second floor hallway and I will come out to meet you at the couch area.
Green Lake: You may wait in the Tangelo waiting area.

Protocol:

• Masks or face coverings are optional in the building and during the entire treatment. If you prefer to wear a mask and/or prefer I wear a mask, please let me know and I am happy to comply.

50-Minute Sessions:
If you are a new patient, please complete the required paperwork ahead of time and bring it to your initial visit. Download the forms here.
